Bank Place Medical Centre welcomes new patients.

All new patients are required to complete the “New Patient Form”. You are welcome to download the form and complete the Patient Registration form and bring with you on the day of appointment.  Otherwise if you could arrive 5-10 minutes before your appointment so your details can be registered. Please make sure you bring your Medicare card, and your Health Care or Pension cards.
